What Is Chronic and Hormonal Acne?

Acne vulgaris is almost always approached as a local skin issue when its biology is systemic. Three mechanisms drive most persistent acne, and they work together in ways that mean addressing only one produces incomplete results.

NF-kB-driven inflammation activates sebaceous glands directly. Elevated IL-1b and TNF-a, the same inflammatory cytokines driving systemic disease, express through the skin by triggering sebum overproduction and follicular inflammation. Androgen and insulin dysregulation amplifies this: androgens drive sebum production, and elevated insulin and IGF-1 from high-glycemic diets amplify androgen signaling in sebaceous tissue. This is the mechanism behind the clinical observation that sugar worsens acne and that improving insulin sensitivity consistently improves skin. Gut dysbiosis is the third driver, allowing inflammatory signals to reach sebaceous glands through systemic circulation. The gut-skin axis is well established, and probiotics and gut repair directly improve acne outcomes in published clinical evidence.

The result is a condition that lives in the skin but is driven by the gut, the endocrine system, and the immune system simultaneously.

Who the Acne Protocol Is For

This protocol is built for patients who are dealing with one or more of the following:

  • Persistent inflammatory or cystic acne that has not fully resolved with topicals, antibiotics, or dietary changes

How the Acne Protocol Works

The baseline panel for this protocol includes fasting insulin, IGF-1, sex hormone panel, inflammatory markers, Vitamin D, and where indicated, a gut assessment. The protocol moves through three stages targeting the inflammation, insulin environment, and gut-skin axis simultaneously.

What the Protocol May Include: berberine for insulin and IGF-1 reduction and gut microbiome reshaping; zinc at therapeutic dose, which has clinical evidence comparable to low-dose antibiotics for P. acnes reduction without microbiome disruption; omega-3 fatty acids for prostaglandin-driven sebaceous inflammation reduction; multi-strain probiotics for the gut-skin axis; and Vitamin D for sebaceous gland regulation. For hormonal acne patterns, DIM to shift estrogen metabolism toward less androgenic pathways, and inositol for patients with PCOS features. As the protocol advances, injectable vitamins at the clinic deliver glutathione for follicular oxidative stress reduction, Vitamin C for collagen synthesis and post-inflammatory hyperpigmentation prevention, B5 for sebum production reduction, and injectable zinc at therapeutic levels. Why does diet affect acne at a biological level?

Genetics determine sebaceous gland sensitivity; insulin triggers its expression. The insulin spike from high-glycemic foods amplifies androgen signaling in sebaceous tissue within hours of eating. Lower insulin means lower IGF-1 and reduced sebaceous activation. This is not a loose association. It is a direct, measured hormonal mechanism.

Why does acne worsen before my period?

In the premenstrual week, falling estrogen reduces its counter-androgenic effect while rising progesterone increases sebum production. DIM and inositol in Stage 2 specifically address this through estrogen metabolism support and insulin sensitivity improvement in hormonal tissue.

How long until I see improvement?

Most patients notice reduced new lesion formation within four to six weeks of consistent Stage 1 adherence. Post-inflammatory hyperpigmentation takes two to three months to fade. Full skin quality improvement, including texture and tone, develops over three to four months of consistent protocol.
